Southwest kindergarten registration opens Feb. 1

The Southwest Local School District will begin kindergarten registration for the 2018/2019 school year after Thursday, Feb. 1. To enter kindergarten, a child must be 5 by Sept. 30, 2018.
Kindergarten registration can be completed online.

Detailed instructions regarding the online application is available on the district’s Website. Parents/guardians also may pick up a copy of the instructions at one of the elementary schools or at the District Office after Thursday, Feb. 1.  

For convenience, parents/guardians can pick up an instruction packet or use a computer at a school building during these conference nights when the building will be open in the evening:
■Feb. 1 and 7, Crosby Elementary: 4-8 p.m.
■Feb. 6 and 8, Harrison Elementary: 4:15-7:30 p.m.
■Feb. 9 and 11, Miamitown Elementary: 4-7:45 p.m.
■Feb. 6 and 8, Whitewater Elementary: 4:30-8 p.m.

Parents/guardians also must print a copy of the Ohio Health History from the district Website or stop by one of the schools or district office to pick up the form. Pages 1 and 2 are completed by parents; pages 3 and 4 are signed by a physician; and page 5 is completed by the dentist.  

The Ohio Health History form must be returned to the building the student will attend before Aug. 21, 2018.

A copy of the child’s birth certificate, proof of residency, and any custody order or divorce decree may be uploaded or be presented to the District Registrar, at the District Office.  

Once completed, parents/guardians must contact the District Registrar at District office 230 S. Elm St., Harrison, OH 45030, Phone: 513-367-4139 to make an appointment to complete your registration.

A child is not considered registered for the upcoming school year until the proper documentation is presented to the registrar at the District Office.

State law and the Southwest Local School District requires the following immunizations:
■series of five diphtheria, whooping cough, tetanus (also known as DtaP), four if the fourth is given after the fourth birthday.
■series of 4 Polio
■series of three Hepatitis B
■two MMR (measles, mumps, rubella, two chicken pox vaccine (varicella)
According to State Law, children who do not meet immunization requirements by the 15th day of school cannot be permitted to attend school and will be excluded upon the 16th day of school.

Bus transportation is provided for kindergarten bus students to and from school in accordance with the law of the state of Ohio.
