The Southwest Local School District will provide two kindergarten registration opportunities for parents of children who need to be enrolled for fall 2013.

Parents of perspective kindergarten students are encouraged to stop by the school their child will attend to pick up an information packet. Kindergarten registration will be held on parent teacher conference nights at each building.

Feb. 7 and 13, Crosby Elementary - 4:30-7:30 p.m.

Feb. 5 and 7, Harrison Elementary - 4:30-7:30 p.m.

Jan. 31 and Feb. 6, Miamitown Elementary - 4-7:45 p.m.

Feb. 5 and 7, Whitewater Valley Elementary - 4:30-7:30 p.m.

To enter kindergarten, a child must be five (5) by Sept. 30, 2013. A birth certificate must be presented at registration or the child will not be registered. If you do not have a birth certificate for your child, send for it immediately. Also, a certified copy of any child custody order or decree must be presented at time of registration.
Registration may be picked up at the office. Fill in the form and bring it with you to registration.

The health record should be turned in at registration, if possible, but must be turned in by Aug. 19, 2013.

State law and the Southwest Local School District requires the following immunizations:
series of five diphtheria, whooping cough, tetanus-also known as DtaP; (four if the fourth is given after the fourth birthday;
series of four polio (three if the third is given after the fourth birthday);series of three hepatitis B; two MMR (measles, mumps, rubella), two chicken pox (varicella)
According to state law, children who do not meet immunization requirements by the 15th day of school cannot be permitted to attend and will be excluded upon the 16th day. Bus transportation is provided for kindergarten bus students to and from school in accordance with state law.

If for some reason, you cannot attend pre-registration, it is necessary to bring in to the school office the completed form and birth certificate by registration time.
